2016 Bid Road Resurfacing and Patching for Holly Forest

Sealed bids for the Holly Forest Association, Inc. for road resurfacing and patching in various areas of the development will be received by April 15, 2016 at the office of the Holly Forest Association located in the Red Bird Golf Pro Shop until 5 o’clock pm (EST) and then opened by the Holly Forest Association Inc. Board of Directors on April 19, 2016 at 9:30 a.m. The sealed envelope should be plainly marked:

“2016 Bid Road Resurfacing and Patching for Holly Forest”

The information for bidders, plans, specifications etc. may be picked up, dropped off or mailed to the following location:

Bidders are encouraged to visit the project locations on their own time to familiarize themselves with the project sites and conditions. Submission of a bid indicates the Bidder’s understanding and acceptance of the site constraints and project requirements.

The successful BIDDER is required to provide the Holly Forest Association with a certified insurance certificate naming Holly Forest Association, Inc. as additional insured. The minimum policy limits and types of insurance are as outlined below:

a. Workmen’s Compen-sation Insurance

b. Commercial General Liability

Limits of Liability

$1,000,000 per occurrence $2,000,000 aggregate

Completion of patching/ paving must be within 90 days from the date of the contract.

If patching/paving is not completed within 90 days of the contract date, a penalty of $100.00 per day will be imposed, unless extension has been agreed upon by the Holly Forest Board of Directors.

Bid must include price per ton for asphalt. Price submitted will remain firm for 120 days. Any work that must take place after this time must be reported to the Holly Forest management immediately so that approval may be obtained by the Holly Forest Board of Directors for continuance.

PATCHING PROCESS

- Saw cut designated areas to be repaired

- Remove damaged asphalt and dispose of it

- Bring sub-grade with crusher run if needed 2” below finish grade, compacting base

- Fill the void with binder asphalt and compact to surface level

RESURFACING

EXISTING ROADS

- Edge roads

- Clean and tack surface

- Resurface road with 2” of S95 hotmix asphalt compacted to 1 ½”

- 1 Gold center stripe

- 2 White side striped
